MAIL THESE 8 FORMS TO THE SCHOOL, OR YOU MAY BRING THEM INTO THE SCHOOL OFFICE FROM AUGUST 1-12.

A MATERIALS FEE OF $75.00 FOR ALL STUDENTS IS PAYABLE TO LCMCS AT TIME OF REGISTRATION.

BRING TO SCHOOL (required by law for public school registration):
  1. Provide proof of child's age - includes a copy of birth certificate or passport
