SEND US YOUR PREVIOUS SCHOOL RECORDS. Use this form to get them to us: UPDATE YOUR IMMUNIZATION RECORDS. This record should arrive with your child's records from a previous school. Come to the office to update them with us. This is required by law. To help you understand the requirements for vaccinations, read: DOWNLOAD AND FILL OUT COMPLETELY FOR EACH CHILD:

MAIL THESE 7 FORMS TO THE SCHOOL, OR YOU MAY BRING THEM INTO THE SCHOOL OFFICE FROM AUGUST 1-12.

A MATERIALS FEE OF $75.00 FOR ALL STUDENTS IS PAYABLE TO LCMCS AT TIME OF REGISTRATION
